The book is well written and certainly brings new life to an old paradigm. However, it is not very balanced.While the Warburg effect is an interesting window into how some tumor cells work, recent studies have shown a "reverse Warburg" effect or “two-compartment tumor metabolism" with several types of tumor, most notably breast and certain brain tumors. In these cases, ketones actually *increase* the size of the tumor and lactate causes it to metastasize (Bonucelli 2010, Martinez 2012). The link between red meat and cancer due to their nitrosamine/nitrile content has also been well established, and not only in colorectal cancer, though certainly unprocessed red meat seems to have less of an impact than processed, and organ meats seem to be beneficial in many ways that muscle meat is not, so this is certainly not clear cut.Tumors with low ketolytic enzyme expression may be unable to metabolize ketone bodies, thus predicting a better response to the ketogenic diet, but cancer is not binary. And it is important to remember that mice are not small furry humans -- their life cycles are much shorter and in experiments their genetics are usually homogenous, unlike in human populations. And forget about cell lines in petri dishes. Even in those scenarios, the results are not clear cut. Cancer cells use many sources of nutrients for fuel, and they change phenotypes often, allowing them to use different fuels as they grow and adapt. There is a mechanism behind this: recent high profile study found that a genetic mutation called BRAF V600E allows cancer cells to use ketones to grow faster. This mutation is present in 50% of melanomas, 10% of colon cancers, 100% of hairy cell leukemias, and 5% of multiple myelomas (Xia et al. 2017). Cancer is smart. Cancer is tough. And cancer did not earn its title as the emperor of all maladies without reason.As with most things in health, human physiology is rarely black and white -- there are countless shades of gray, a whole color spectrum, not to mention UV and infrared variables that we have not discovered yet. While I applaud the book's rebellious spirit and compelling narrative, a lot of folks are grabbing on to this approach desperately looking for a magic bullet.When asked, “Does the ketogenic diet beat chemo for all cancers?” on the Tim Ferris podcast episode #188, Dominic D’Agostino said the following:‘”Absolutely not… A number of situations where the ketogenic diet may not be the preferred therapy for most cancers, I would say, leukemia, lymphomas, Hodgkin’s lymphoma, thyroid cancer, testicular cancer, if caught early prostate cancer, melanoma, breast cancer. All these cancers can be effectively treated with chemotherapy or radiation in some cases, and also brain tumors if it’s grade 1 or 2 tumor that’s not very metastatic and is more localized then surgery, radiation, and chemo can be very effective.”We still do not have a clear answer.
